Plenty of women have been told that underwire is harmful, and specifically that it causes breast cancer. There is no credible evidence for this, and it has been examined repeatedly. MD Anderson Cancer Center lists it among the breast cancer myths that do not hold up.
It is worth knowing why the myth is so sticky, because the reason is reasonable. A wire is metal, it sits close to the breast, and when it hurts it hurts in a way that feels significant. The mind fills in a mechanism. But a wire is a rigid arc doing a structural job: it holds the shape of the cup so the fabric does not have to, which is why an underwired bra can support a full bust with less bulk than a wireless one. A bra helps upper and middle back ache caused by breast weight being carried badly. It does nothing for lower back pain, disc problems, injury, or back pain from any other source. If your pain is in the lower back, a bra is not the tool.
Try this to feel the mechanism. Hold a heavy book flat against your chest — easy, you could stand like that for a while. Now hold the same book out at arm's length. Within seconds your upper back is straining, and nothing about the book changed. Only the distance from your body changed, and that is what your back has to counterbalance.
A bra does the same job in miniature. A snug, level band keeps breast weight held close to your ribcage, where your skeleton takes it. A slack, stretched or riding band lets that weight sit lower and further forward, and your upper back spends the day pulling against it.
